For the service settings detailed below, PhysioFunction is responsible for the day-to-day support of this agreement. This agreement outlines the parameters of all physiotherapeutic and related services, equipment supplies, medical-legal and training services that are included by directors. This agreement does not replace existing procedures and procedures unless expressly stated. This agreement was last amended on September 10, 2019. The objective of this agreement is to manage client expectations regarding our physiotherapy and related services, equipment supplies, medical,legal and training services, and to ensure that we maintain a high level of service. Effective management of physiotherapy performance is the result of maintaining a consistent level of service. The following sections contain relevant details on service availability and complaint handling. The coverage parameters covered in this contract are: The Service Supervisor client is responsible for the periodic verification of this document. The content of this document may be amended if necessary, provided that the directors are solicited by mutual agreement and are communicated to all parties concerned. The holder of the document will take into account all subsequent revisions and will request, if necessary, reciprocal agreements/authorizations.
The objective of this agreement is to ensure that the appropriate elements and obligations are in place to provide consistent physiotherapy and related services, equipment supplies, medical and training services, assistance and provision to physioFunction`s client (s). This agreement remains valid until it is replaced by a revised agreement approved by the directors. This agreement is a service level agreement (“ALS” or “Agreement”) between PhysioFunction and our clients for the provision of physiotherapy and related services, equipment supplies, medical services and training. . Client Services Supervisor: PhysioFunction Review Period: Annual Previous Review Date: 09th July 2015 Next Review Date: 08th July 2016 This agreement is valid from the validity date described above and valid until further notice. This agreement should be reviewed at least once every 12 months; However, instead of a revision during this period, the existing agreement remains in force.
